Notice of Claim Against Maricopa Special Health Care District
This form is provided to assist in presenting a claim to Maricopa County Special Health Care District that complies with the requirements of A.R.S. § 12-821.01, which defines the requirements for filing a claim against a public entity in the State of Arizona. It is important to complete all applicable items on the form in order to assure compliance with state law. Failure to do so may result in your claim being rejected. Filing a valid claim will always remain your sole responsibility.

Please Note: Maricopa County Special Health Care District was formed on January 1, 2005. Prior to that date Maricopa Medical Center and Maricopa Integrated Health Systems were administrative subdivisions of Maricopa County. The District and the County are separate and distinct political subdivisions of the State of Arizona. Service on the District does not constitute service on the County for any purpose, and service on the County does not constitute service on the District for any purpose. If you wish to preserve any claim that you may have against either the District or the County, you must serve a notice of claim on that entity.
